Participation waiver

Assumption of Risk & Release of Liability Motion Is Life Fun Run — October 3, 2026 By registering for the Motion Is Life Fun Run, I acknowledge and agree to the following: Assumption of Risk I understand that participation in an outdoor walking and running event involves inherent risks, including but not limited to uneven terrain, weather conditions, physical exertion, and the presence of animals. I voluntarily assume all such risks. Animal Risk Acknowledgment I understand that this event is pet-friendly and that adoptable animals from Pet Haven Inc. of Minnesota will be present on-site. I acknowledge the inherent risks associated with the presence of animals, including my own pet if I choose to bring one, and I assume full responsibility for my own animal's behavior throughout the event. Physical Fitness Certification I certify that I am in adequate physical condition to participate in this event and that I have not been advised by a medical professional to avoid this type of activity. Medical Treatment Authorization I understand that this event does not have medical staff on-site. A volunteer with basic first aid knowledge may be present, but is not a substitute for professional medical care. In the event of an emergency, event staff will contact emergency services on my behalf. I acknowledge that Cadence Chiropractic, PLLC, event volunteers, and any volunteer support personnel are not responsible for providing medical treatment beyond contacting emergency services. Release of Liability I release and discharge Cadence Chiropractic, PLLC, Pet Haven Inc. of Minnesota, event volunteers, property owners, sponsors, and their respective employees and agents from any and all claims, demands, or liability for injury, loss, or damage arising from my participation in this event, to the extent permitted by Minnesota law. Indemnification I agree to indemnify and hold harmless Cadence Chiropractic, PLLC, Pet Haven Inc. of Minnesota, and all associated parties from any claims arising from my participation, including claims related to my pet's behavior or actions during the event. Minors If I am registering on behalf of a minor under the age of 18, I confirm that I am the parent or legal guardian of that minor and that I accept these terms on their behalf. Event Rules I agree to follow posted route markings, event guidance, and the direction of event staff and volunteers throughout the event. Photo & Media Release I grant permission for photographs and video taken during the event, which may include me and my pet, to be used by Cadence Chiropractic, PLLC and Pet Haven Inc. of Minnesota in community recaps, social media, and promotional materials. Acknowledgment I confirm that I have read, understood, and voluntarily agree to the terms of this waiver. I understand this is a legally binding document governed by the laws of the State of Minnesota.
